Player Registration Requirements

If a Bethel Park Football player plans to participate this season, all required district forms and registrations must be completed before practice or competition.

Please review and complete each item below:

Healthy Roster

All student-athletes must register and upload required documents through Healthy Roster.

  1. Create or log into your account
  2. Complete all required athlete information
  3. Upload completed physical paperwork
Bethel Park School District Activity Fee

The district activity fee must be paid prior to participation.

  1. Review fee requirements
  2. Submit payment as directed by the district
Bethel Park School District Physical Forms

All required PIAA physical forms must be completed and submitted.

  1. Download required forms
  2. Have physical completed by a licensed medical provider
  3. Upload completed forms to Healthy Roster
